Estate Plan Prep Form
As you begin your estate planning, living trust, it is important to take time to evaluate the decisions you would like to make. It is important to know that this is a living document that you can change in the future at any time while you are alive. Before your estate analysis session, please take time to review the list below in detail so that you are prepared to work with your advisor efficiently.
Begin to think about the legacy you want to build. Who would you like to be your beneficiaries and the people you would like to act on your behalf. Please know their first name, last name, date of birth, address, and phone number. (Address and phone number are not required but highly recommended)
If you have minor children: Who is the first person you would like to care for and raise your minor children should you pre-decease them? < If that person is not willing or able then Who? < If both persons are not willing or able then Who?
If you own one or more properties please have a copy of the current Property Deed(s) for each property. If you do not have it we may be able to help find it with our real estate department but it takes longer to complete your estate planning document.
If there are any checking, savings, or investment accounts that will be titled in the name of the trust please have those account numbers with approximate values listed for us to prepare the appropriate documents. (Please note that we have the information regarding any investment accounts you have with us)
Who is the first person you would like to handle financial matters such as banking accounts, investment accounts and life insurance? < If that person is not willing or able then Who? < If both persons are not willing or able then Who?
For each Spouse Separately: should you become incapacitated; who is the first person you would like to direct regarding health care matters? < If that person is not willing or able then Who? < If both persons are not willing or able then Who?
For each Spouse Separately: would you like to be cremated?
For each Spouse Separately: do you have any special item request you would like to leave to any specific person? (watches, jewelry, etc.) Please have an idea of those items along with the persons names.
Lastly, please list any questions you may have so that we may address at the time of your estate analysis.
